Client Liaison Nurse 

Angela Hamlin Health Care Professionals are an Independent Nursing and Care provider, offering personalised, professional domiciliary health care. Services include the provision of nurses and/or carers, ranging from short daily visits to longer day duties, as well as live-in residential options. About the role

This is an opportunity to join Angela Hamlin Health Care Professionals as a vital member of our team as Client Liaison Nurse & Clinical Appraisals Officer. You will be responsible for assessing our clients, understanding their needs, and explaining the types of care and services available to them from Angela Hamlin Health Care Professionals. You will assist in the clinical governance of our services by carrying out regular supervisions and appraisals of our care staff, ensuring they are providing care in line with the clinical code of conduct and current CQC regulations and standards.

Responsibilities

  • To assess clients and understand their care needs.

  • Explain the types of care and services available to them from Angela Hamlin Health Care Professionals.

  • Liaise as necessary with allied health professionals (e.g. GPs, district nurses, Occupational Therapists, hospitals etc) with the goal of delivering holistic, client-centred packages of care.

  • Create and maintain client care plans. 

  • Carry out risk assessments for all new clients

  • Coordinate with carers, nurses and other staff members.

  • Conduct staff supervisions and reviews regularly, providing feedback and identifying any concerns.

  • Staff training where necessary.

Essential

  • Experience as a Registered Nurse.

  • Excellent organisation and communication skills.

  • Respect for client confidentiality and discretion at all times.

  • Adhering to company policies and procedures at all times.

  • Valid NMC pin.

Desirable

  • Experience within a community or domiciliary care setting.

The salary for this role will be £40,000 per annum; this will be pro-rata if part-time.
