Our priority is you and your family.
Safe care at home means more than simply arranging practical support; it means surrounding every client with trusted professionals who understand the client’s health, their routines, and what matters most to them.
Our nurse-led team work closely with clients and families to create tailored care that feels safe, dignified, and genuinely personal. From the first conversation, the focus is on clear guidance, clinical understanding, and calm, confident decision-making, so that families know they are never facing these choices alone.
Meet Our Client Liaison Team
At the heart of everything we do is our Client Liaison Team; a group of experienced, compassionate nurses dedicated to making every stage of the care journey smoother, clearer, and completely supported.
From the very first phone call, their goal is simple: to make your experience easier, calmer, and guided by expert knowledge.
At the heart of our service is a deep commitment to advocating for every client we care for. We believe that exceptional care goes beyond meeting physical needs. This means being a constant voice for our clients’ preferences, dignity, and wellbeing. This advocacy is built into every layer of our organisation, ensuring that our clients are truly seen, heard, and supported in every aspect of their care journey.
Empathy
We understand that needing care, whether suddenly or over time, can feel overwhelming, emotional, and confusing. Families are often faced with difficult decisions, complex options, and a lack of clear information. That’s why we make it our mission to remove the pressure from clients and families by guiding them every step of the way.

How We Work
Our team offers honest, expert advice on funding and care options, and what to expect - without pressure or obligation.
Care Planning Support
We guide families through the full process of arranging care, ensuring every medical, emotional, and lifestyle need is carefully understood and met.
Liaison with Medical Teams
We work directly with GPs, physios, consultants, hospital discharge teams, and private clinics to coordinate care and keep communication seamless.
Paperwork & Practicalities
From documentation and care agreements to medication records and rotas, our team handle all the practical details so that families can focus on what matters most.
Availability & Reassurance
Our nurses are available 24/7 to answer questions, provide reassurance, and offer clarity when choices feel difficult or uncertain.
A Nurse-Led Approach
Our nurse-led management team ensures that a client’s health and wellbeing are monitored, understood, and prioritised. Our team work closely with loved ones, and families, liaising with healthcare professionals to ensure the right mix of care and support is delivered.
Every individual care plan is designed to reflect not just clinical needs, but also personal routines, preferences, and values. As medical situations evolve and change, our Client Liaison Nurses adapt care packages, maintain regular contact, and ensure clients’ voices are always central in all the care decisions.
Our Client Liaison Nurses also support and encourage our care staff to be advocates - through in-depth training - led by our in-house Professional Development Manager, Hazel. This training includes not only essential clinical skills but also communication, compassion, safeguarding, and dignity in care. Continuous supervision, and professional development ensures advocacy remains a living part of daily practice, not just policy.
